Course Re-take Application Form

   If you have failed a course and would need to retake it, please fill in the Retake course application form, get it signed by your supervisor and the students affairs office. Submit one copy to the course instructor and another to the Students Affairs Office. IT IS THE ONLY WAY TO BE ENROLLED AGAIN IN THIS COURSE. Normal process such as selecting the course as new course WILL NOT WORK. Please complete this process within one week from the beginning of the course. Failure to do so may cause your marks not uploaded onto the system.
